Name: First then last, as always.
Alias/Nickname: What is it, if they have one?
Age: What they appear to be and how old they actually are.
Gender: Only has to be a single word.
Height: Feet then inches
Number: You should know what this means ^-^
Original Hollow Type: Their original Hollow type/appearance.
Appearance: At least 300 words, and be detailed.
Weapons: Non-zanpakuto weapons, of course
Zanpakuto: Sealed, Shikai, and how your appearance changes when your zanpakuto is released.
Mask: What does your remaining bit of mask look like? Where is it (jaw, top of your head, etc.)?
History: At least (please do more than this if you can) 400 words
